This overview covers all the steps involved in both the application and in the removal of lockout/tagout.

Do the following steps according to the specific provisions of the employer's energy-control procedure:

  1. Prepare for Shutdown.
    Review the procedures for the equipment to be serviced. Before turning off the equipment, the authorized employee must know:
    • The type and magnitude of the energy.
    • The hazards involved.
    • The means to control the energy. This information is found on the equipment-specific lockout/tagout procedure document.
    • Additionally, all affected employees should be informed of the shutdown.
  2. Shutdown.
    Turn off equipment according to the equipment’s specific procedures.
  3. Isolate.
    Locate all energy-isolating-devices and isolate the equipment from its energy source(s).
  4. Apply Lockout/Tagout.
    Attach lockout/tagout devices to each EID. The lockout devices must be attached in a way that secures the EID in the “safe” position. The tag must contain the name of the person who performed the lockout and any additional information.
  5. Release/Block Stored Energy.
    All potentially hazardous stored or residual energy must be released, blocked, disconnected, restrained, or otherwise rendered safe. If a possibility exists for reaccumulation of hazardous energy during service/maintenance, it must be monitored.
  6. Verify Isolation.
    Before any work begins on the equipment, the authorized employee must test and verify that the equipment has been properly isolated and deenergized. If the energy isolation is not successful, a machine/equipment hazard analysis must be RE-CONDUCTED before work can begin on the equipment.
  7. Perform Work.
    Complete required service/maintenance work. Follow special procedures if the task is incomplete and the shift changes or if the equipment requires repositioning or testing.
  8. Prepare for Lockout/Tagout Removal.
    Prep the area and equipment for reenergization. Follow the lockout/tagout procedure to ensure that the equipment is fully intact and ready to be operated again, controls are in a neutral position, all nonessential items have been cleared, and all personnel are safe and accounted for.
  9. Remove Lockout/Tagout.
    Remove locks, tags, and energy-isolating devices.
  10. YOUR LOCK IS YOUR LIFE. All locks and tags must be removed by the same person who applied them.
    **NOTIFY AFFECTED EMPLOYEES that the equipment is no longer under LO/TO BEFORE reenergizing the equipment.**
