Lockout/tagout empowers workers to have exclusive control over the protection of their own life. With LO/TO you are responsible. You hold the literal key that is protecting your life and health. Locking out equipment ensures that you are not still working on it when it is reenergized.
If the equipment is reenergized while you are still working on it, you could be seriously injured or killed.
The numbers are staggering: Each year lockout/tagout(LO/TO) violations lead to an estimated 120 fatalities and 50,000 injuries. Lockout/tagout violations are consistently among OSHA’s Top Ten common violations. Without proper LO/TO procedures, approximately three million workers are at risk every day.
You must personally attach a lock to each point of energy isolation for the job. This ensures that the system cannot be reenergized until you are out of harm's way. There should be as many locks on the system as there are people working on it. If a maintenance job requires 3 workers, then 3 locks should be present.
Each lock should only have one key (no master keys or duplicate keys are allowed).
YOUR LOCK SHOWS THAT YOUR LIFE ON THE LINE. Never allow anyone else to remove it for you. If you applied the lock, you must be the one to take it off.
NEVER try to activate an EID that has a lock or tag on it or ever remove someone else’s lock or tag. Those devices are protecting the person's life.
Never work under someone else's lock (e.g. crew chief's lock).
